Suggested approach

Prepare the correct stage
Finish the capture stage, Plagued Lands, and accept the new quest before hunting. This stage requires kills instead of another trapped bear.
Follow the named objective
Check the exact Rabid Thistle Bear name and watch the counter after the first kill. Work through manageable pulls until the tracker shows all twenty; ordinary bears are not the listed target.
Confirm credit and hand over
Return to Tharnariun only after the counter is complete. Restock before checking any follow-up, since completing this quest does not automatically complete the rest of his chain.

Planning notes

The capture task introduces the disease, while this stage measures kills. Track only the exact named infected bear and inspect the first credit before committing to a longer hunt. This prevents an ordinary bear circuit from wasting the outing.

If progress stalls

Verify that Cleansing of the Infected has been accepted after Plagued Lands. Reusing the capture tool does not advance a kill counter.
